    What to expect:

    The

    primary responsibility of the Access Center Crisis case manager services for

    clients who are referred to the Access Center for crisis intervention

    services.

    Some responsibilities include:

    • Provide

      short-term transitional community and center-based crisis case management to

      eligible Bert Nash clients who do not have an active case manager until a case

      manager is assigned. To help crisis clients to understand the symptoms

      of their mental illness and to minimize the effect of those symptoms in all the

      significant areas of their lives and to develop an individualized crisis plan

      with client.
    • Provide

      1 on 1 support for crisis client awaiting inpatient hospitalization.
    • Provide

      follow up contact (telephone and face to face) for clients who are diverted

      from hospitalization and safety planned.
    • Conduct

      brief phone assessments to determine the level of acuity on incoming calls to

      the Access Center
    • Assist

      with referrals, the processing of paperwork and data entry associated with the

      screening and hospitalization of clients.
    • Provide

      supportive counseling, solutions focused interventions; to increase client's

      ability to stabilize symptoms during a crisis and function in all life domains.
    • Providing outreach and crisis

      intervention services to clients and prospective clients to enable timely

      access to those services appropriate and necessary for crisis stabilization

      with the purpose of diverting the client from hospitalization and allowing the

      client to maintain independent living.
    • Complete strength assessment with

      client and identify strategies or treatment options associated with the

      consumer's mental illness with the goal of minimizing the negative effects of

      mental illness symptoms or associated environmental stress.
    • Coordinate all clients' services with

      other providers, family, and community collateral primarily with the client

      consulting in the process.

      To be successful in the job, we are

      looking for a person with the following:

      • BA/BS degree or

        four years of equivalent education and/or experience working in the human

        services field.
      • Reliable

        transportation; and a valid, unrestricted driver's license and proof of current

        car insurance

        Position Details:

        Non-Exempt/Hourly

        Minimum starting salary $13.23 per hour
